During the post-processing stage of producing the adsorbent, sodium hypochlorite was added to the water after removing bromine cyanide for treatment purposes, which resulted in the generation of hydrogen cyanide due to a chemical reaction. Since no exposure prevention measures were taken, the worker was exposed to hydrogen cyanide.
